Answer :The Lady Humber Health Centre is a multi-service facility located east of the north shore of Lake Superior. Our catchment area has a population of just over 4,300 which includes the communities of Dubreuilville, Hawk Junction, Michipicoten First Nation, Michipicoten Township-Wawa, Missanabie, and White River.

Answer :The Health Centre includes ten acute care, two respite and sixteen long term care beds. We operate a 24-hour emergency department, staffed by a Registered Nurse and an on-call Physician. We also provide a Surgical Program, Diagnostic and Therapeutic services, Telehealth and a Nurse Practitioner Program.
Question : 3, How many community service programs do you have?
Answer :Our community service program includes North Algoma Counselling Service, North Algoma Diabetes Education Program, and outreach nursing clinics in the communities of Dubreuilville and Missanabie.
Other on-site services that operate as separate entities are the Wawa Medical Centre, Wawa Family Health Team, North East LHIN Home and Community Care and the Paramedic Services of Algoma.

Question : 5, How to raise an issue with LDHC Board of Directors?
Answer :The Lady Dunn Health Centre (LDHC) Board meetings are held on the third Monday of the month with the exception of meetings that fall on a Stat Holiday. The Board meetings are open to the public. Meetings commence at 7:00 pm and conclude at approximately 9:00 pm.
Persons wishing to raise a matter with the Board must put the request in writing at least seven working days prior to the scheduled meeting. The request must be made to the Chairperson of the Board and mailed or delivered to the Administration Office of the Lady Dunn Health Centre. Answer :Dr. A. Oberai - Chief of Staff
Dr. Anjali Oberai has been working in Wawa since 1998. She graduated from the University of Ottawa, along with her husband, Dr. Mike Cotterill, and together they moved to Wawa where they raised two children. Dr. Oberai loves both rural medicine and all that life has to offer in rural Ontario.

Question : 8, Do you have Emergency Services available?
Answer :When medical emergencies occur, Lady Dunn Health Centre is prepared to serve. Each year the LDHC emergency department addresses over 6000 visits. The emergency services department provides 24-hour emergency nursing coverage, an on-call physician along with a full complement of medical and technical services and equipment to treat medical emergencies. Patients are seen and assessed by the nurse then treated based on urgency.

Answer :Yes, you may submit a request to access your health records. Your health care provider and the health records staff will assist you throughout the process. Furthermore, you may sign a consent form to give anyone access to your health records if desired. Generally the facility has 30 days to comply with this request.

Answer :Doctors, nurses, and other health care workers who are treating you look at your health record on a “need to know” basis. This is true if the treatment takes place here or somewhere else. Reasons we would share relevant parts of our record include:
• If you are referred to another facility, specialist. Why? Because he or she will need to see what services you have already had before recommending a plan of care.
• If you have Home Nursing services. Why? Because the nurse or case manager may need to check your records to find out about your condition and what medicines you are taking.
• We also may use the information from your health record to help us plan and audit services.
We will send copies, reports to your family doctor and/or the doctor who is handling your care or in the case of Dubreuilville and Missanabie clinic of your care unless otherwise requested by you. You can ask us not to mail or fax the notes made by your health care workers to a particular doctor, or facility.
